Module 4 – Phase 2: Scientific foundations of care-based research (funding provider German Federal Ministry for Education and Research)

Funding for projects in the framework of three thematic fields was applied for, the benefits of which are at the direct forefront of an implementation by the social insurance carriers. This includes, where applicable, projects which focus on essential methodological or theoretical questions of care-based research.

In particular, on the basis of specified deficits or with the help of needs analyses, new approaches for the empowerment of persons with chronic diseases, for patient information and for structural anchoring of patient orientation are developed and tested in a first application.

Additionally, the further development of methods for measuring the effectiveness and sustainability of measures and structures of a participatory arrangement of care are strived for.

• Diabetes mellitus: New paths for optimising general practitioner care (DIANA)
• Development of a procedure for assessing evidence by means of systematic weighting of patient-relevant endpoints – BEPE
• Development and evaluation of a training program to foster the self-management competence of illiterate patients with diabetes mellitus of Turkish origin (SITD)
• Evaluation of dimensions and measurement scales in patient-centeredness
• Improvement of the care of poorly controlled patients with diabetes mellitus type 2 (DEBATE)
• Communication competencies of the chronically ill with regard to the interaction with treatment providers (KoKoPa project)
• Hospital behaviour of the chronically ill
• OptRisk: Optimisation of risk consultancy through presentation of changeability of individual life expectancy
• Patient competence and illness-related worries (progression anxiety) of tumour patients in the course of oncology rehabilitation. A contribution to construct validation.
• Patient-oriented consulting in the case of rare diseases – A modularised concept for the improvement of quality of life of patients with achondroplasia
• Are psychiatric patients motivated to actively support shared decision making? Development and validation of a measurement instrument
• Improvement of home-based care of palliative patients through support of care-giving family members
